Before Your Appointment
- Check Your Email
You’ll receive a welcome email from 1 Alliance Counseling & Psychotherapy Services powered by Simple Practice with a link to your Client Portal. - Complete Your Intake Forms
- Log in to your Client Portal.
- Fill out and sign all required forms (Demographics, Client Service Agreement, payment authorization, etc.). The Release of Information form is optional. However, if you are participating in couples or family counseling, you are required to complete the Release of Information form for your significant other / family member.
- Upload a photo of your ID and insurance card (if applicable) in the client portal.
- Please remember to complete and submit online forms prior to your first appointment.
- Confirmation of Your Appointment
- You’ll get an automatic confirmation and reminder with your appointment details.
- For telehealth visits, the secure video link will be included in that message.
- In-Person Appointments
- Arrive 10 minutes early. Please make sure you previously completed all paperwork requested online on the simple practice portal.
- For Telehealth Appointments
- At your scheduled time, click the link in your reminder email or log into your Client Portal and click “Join Video Session.” Enter your name if prompted to access the virtual waiting room.
- Go to “Services – Telemental Healthcare” for complete instructions.
- Please make sure you previously completed all paperwork requested online on the simple practice portal.
